Preface: The body speaks in signals

Medicine, as most people experience it, is a discipline of arrivals. Something has gone wrong — a symptom, a number out of range, an event — and the machinery of care engages to meet it. This is not a criticism. The capacity to respond decisively to disease is one of the great achievements of modern medicine, and nothing in this book argues against it.

But a discipline of arrivals has a blind spot, and the blind spot is time. By the time most conditions arrive, they have been developing for years — quietly, measurably, and largely unattended. The heart attack is sudden; the atherosclerosis behind it was not. The diabetes diagnosis is a moment; the metabolic drift toward it was a decade.

This book is about that decade. It is about the long, quiet interval before arrival — the period during which the body is already speaking, in signals faint enough to miss and specific enough to read, if someone is listening.

Comprehensive longevity medicine is, at its core, the practice of listening earlier. It is not a new technology or a proprietary protocol. It is a shift in when the conversation begins — from the moment of arrival to the long approach that precedes it.

I came to this work as a urologist. That background matters more than it might first appear, and a later chapter is devoted to why. For now it is enough to say that urology trains a particular kind of diagnostic attention — and that this attention, turned toward the question of how a body ages, is the origin of everything that follows.

A word on tone. This book will not promise what it cannot deliver. It will not tell you that aging can be reversed, that any compound is a fountain of youth, or that the future of your health is anything other than a matter of probabilities. What it will do is take those probabilities seriously — seriously enough to measure them, to act on them early, and to be honest, always, about the difference between what is known and what is merely hoped.
